Our nursing services include:
Health assessments & care planning: Comprehensive assessments are conducted upon admission and regularly thereafter to monitor changes in health status. Individualized care plans are developed and updated in coordination with families, physicians, and other care providers.
Medication management: We ensure accurate medication administration and monitoring, including dosage reminders and adherence support, under the supervision of licensed nurses.
Chronic disease monitoring: We assist in managing chronic conditions such as diabetes, hypertension, and heart disease through regular vital sign checks, symptom tracking, and health education.
Coordination with healthcare providers: Our nurses communicate regularly with primary care physicians, specialists, and family caregivers to ensure continuity of care.
Health education & counseling: We empower participants and families through education on disease management, nutrition, medication, and preventive care.
Emergency response: Our staff is trained to recognize early warning signs of health decline and respond promptly to medical emergencies, ensuring participant safety at all times.

Nutritious, culturally-tailored meals: We serve freshly prepared meals that reflect the dietary needs and cultural tastes of the Indian community and others we serve. Each meal is planned to be both satisfying and health-conscious.
Dietary accommodations: Our team accommodates a variety of special diets, including vegetarian, diabetic-friendly, low-sodium, and heart-healthy options. Meals are modified based on individual care plans and dietary restrictions.
Meal schedule: We provide a nourishing breakfast, a hot lunch, and an afternoon snack each day. All meals follow guidelines established by Maryland’s Office of Health Care Quality and are served in a clean, comfortable dining area that promotes social interaction and a sense of community.
Safe & supportive environment: Our staff assists participants who need help with eating, and we monitor food intake to support overall health goals. We also ensure that all food is prepared and handled in accordance with state food safety standards.

Key functions include:
Psychosocial assessment: Conducting initial and ongoing assessments to evaluate the emotional, behavioral, and social needs of each participant.
Care planning & coordination: Collaborating with the interdisciplinary team to develop individualized care plans that address the participant’s health, social, and support needs.
Supportive counseling: Providing individual and group counseling to help participants cope with aging, chronic illness, loss, family changes, or other life transitions.
Family support & education: Offering guidance to families and caregivers on how to manage care at home, connect with community resources, and navigate long-term care planning.
Crisis intervention: Assisting in times of emotional or medical crisis by providing immediate support and coordinating follow-up services as needed.
Resource referral & advocacy: Linking participants and their families to community services, such as housing assistance, legal aid, mental health services, and transportation. Advocating on their behalf to ensure access to necessary supports.
Discharge planning: Coordinating services and support systems for participants transitioning out of the program to ensure continuity of care and safety.

Daily group activities: Including exercise classes, music therapy, arts and crafts, cultural celebrations, games, and reminiscence therapy.
Cognitive engagement: Puzzles, memory games, discussion groups, current events, and reading sessions to support mental stimulation.
Socialization opportunities: Group discussions, birthday and holiday celebrations, community guest visits, and peer-to-peer interaction.
Individualized activities: For participants with mobility or cognitive limitations, we offer one-on-one activities tailored to personal abilities and preferences.
Cultural relevance: Activities are planned with cultural sensitivity and inclusion in mind—celebrating festivals, music, and traditions important to our Indian and diverse participant population.
Recreational outings: When appropriate, we organize supervised trips to local parks, cultural centers, and community events to enrich social experiences.
Activity calendar: A monthly calendar is developed and shared with participants and families, offering structure and consistency while allowing for variety and personalization.
All activities are planned and implemented under the supervision of a qualified Activity Coordinator, in alignment with participants' care plans and in compliance with Maryland Department of Health regulations.

Reliable: We operate on a consistent schedule, providing timely pick-up and drop-off to and from the participant’s residence.
Safe: All drivers are professionally trained, and our vehicles are maintained in accordance with safety regulations and standards.
Compassionate: Our staff assist participants during boarding and exiting, ensuring comfort and personal attention throughout the ride.
By offering transportation, we reduce barriers to care and help ensure regular attendance, which is vital to the health, engagement, and well-being of our participants. This service is especially important for seniors who live alone or cannot drive, allowing them to remain connected to their community and receive the support they need each day.
